A note from the nurse's office
As a condition for attendance, ISG requires that children be immunized against the following diseases:

| Diptheria | Four (4) inoculations including a preschool noculation between ages 4-6 |
| Tetanus | Four (4) inoculations including a preschool noculation between ages 4-6 |
| Polio | Four (4) inoculations including a preschool noculation between ages 4-6 |
| Measles | One (1) and a second inoculation between the ages of 14-16 (if not previously given) |
| Mumps | One (1) and a second inoculation between the ages of 14-16 (if not previously given) |
| Rubella (German Measles) | One (1) and a second inoculation between the ages of 14-16 (if not previously given) |
| Tuberculosis | BCG Vaccine or TB skin test within the last 12 months |
| Meningococcal | One (1) within the last 3 years prior to enrolling and every subsequent 3 years |
